Medical Home Maintenance Program
Welcome to our practice. We are honored to be your medical home and we are committed to providing you with the best care we can. Our hope is that we form a partnership to keep you as healthy as possible, no matter what your current state of health. We will share our medical expertise with you, and we hope you’ll take responsibility for working toward the healthy lifestyle that is so important to your well-being. Few of us have a completely healthy lifestyle, but each day we can take a step closer to a healthier life. You can expect evidence-based care from your clinician and team, as well as support for self-management of your health and health care. It will give us great pleasure to work with you on these goals, either through our own expertise, through reading we might give you, or by referral to other health professionals.We want everyone to be involved in our health maintenance program. Everyone who joins our practice should start by having a complete physical exam followed by periodic check-ups to test for a few specific diseases. To effectively serve you we will ask for a complete medical history, list of medications, and a list of your specialist's names so that we can have a complete picture of your health and coordinate care effectively. We will track hospital visits, specialist visits, and emergency room visits to coordinate your care effectively.
If you need clinical advice during office hours, please call to leave a message, or send a message via our secure portal. Your call will be returned same day in most cases. However if it's an emergency please do not leave a message! We really mean it when we say call 911 if it's an emergency! We clearly post office hours for each location on the website. After hours care is available at 2 of our locations' urgent care clinics. By using an urgent care affiliated with us, all of your health history is available to the urgent care practitioner. Access to a doctor after hours by phone is also available by calling the main phone line for your physician. There is a choice to connect to the answering service, who will put you in touch with a physician by phone after hours if needed.
We look forward to working with you as your family doctor. Please contact us whenever you’d like to talk about anything you think may be affecting your health. It’s our hope that we can have a relationship where the lines of communication are open and communication goes both ways. Let’s work together to help you live the satisfying life that you deserve.
